To report an incident or accident, download this form into Adobe Reader then complete the details in accordance with the instruction, select your Area Office and select the “Submit to Area” button to create an e-mail in your local e-mail account. Then send the e-mail to the address in the “to field”.
This is a trial process for Southern Area which in due course will be extended to all areas and reporting points once the system is proved effective.
